Parent or guardian will sign below, indicating his/her agreement with the terms of this document and giving permission for ARTIST to participate in the May 2011 exhibit at VALISE Gallery.
ARTIST agrees that his/her Work will be displayed from May 6, 2011 through May 29, 2011 at VALISE Gallery 17633 Vashon Highway SW (just south of Express Cuisine). ARTIST will:
• Deliver Work to VALISE Gallery
o Monday May 2 - 4:00 p.m. to 8:00 p.m. (or)
o Tuesday May 3 - 8:00 a.m. to 1:00 p.m.
Include attachment hardware for wall or ceiling hanging, and written technical instructions for installation of work. If work requires specialized equipment (video monitor, etc.), please provide.
• Pick up Work (if not sold) from VALISE Gallery:
o Monday May 30 - 4 p.m. to 8 p.m. (or)
o Tuesday May 31 - 8:00 a.m to 1:00 p.m.

INSTALLATIONS, READINGS, PERFORMANCES, “LIVE” TEMPORARY WORKS are welcome and encouraged. E-mail description of Work to info@valisegallery.org by midnight April 29, or call Elizabeth Conner at 206.310.0251.

VALISE collective members will display and/or promote Work as they see fit. All Work will be insured by VALISE while in its possession at the Gallery.
Work may be for sale or “not for sale” (NFS). To help cover operating expenses for the month of May 2011, and to pay for insurance coverage for the artwork, VALISE will take a commission of 50% on all sales of Work on display in the VALISE Gallery during May 2011. Please price your Work accordingly. (To determine price of Work, multiply desired “take home” dollars by two.) All funds owed to ARTIST for sales of Work will be paid by VALISE on or before June 15, 2011.
VALISE shall be responsible for the collection and payment of all sales taxes on sales of ARTIST’s work. ARTIST shall be responsible for payment of all income taxes imposed upon ARTIST's share of the sales proceeds received by ARTIST under this agreement, and VALISE shall not be responsible for any withholding or reporting with respect to such taxes.
